Circulation Authorization Request

Use this request form to authorize another person to use your ID. This will allow that "proxy patron" to check materials out in your name. The loan period and any fine or fees will be those that apply to you.

You may authorize more than one other person to check out material for you. Please fill out one form for each individual.

The proxy patron must present their ID for each transaction.
